Dual Credit Lab Safety Training Confirmation

All ASU dual credit instructors who teach a lab must fill out this form indicating that all students have successfully completed lab safety training each semester. Documentation must be submitted, such as copies of PowerPoint presentations, quiz questions, links to training videos, etc. If an instructor teaches multiple courses in a semester, this form must be submitted for each course being taught.

You will be required to upload a roster of your students who completed the training. Instructions on printing/viewing your roster in Ramport can be found on the Instructor Resources website. You may also just upload a list of student names.

I verify all students enrolled in this dual credit course have successfully completed lab safety training as required by Angelo State University. All students have passed an assessment showing their knowledge of lab safety prior to participating in a laboratory setting.
